This agreement is between the service provider and the customer named above. By signing below, the customer agrees to the following terms.
Payment. Payment is due upon completion of each visit at the agreed rate. Recurring services are billed at the same rate unless otherwise agreed in writing.
Scheduling. Services will be performed on a recurring basis as agreed. The provider will make reasonable efforts to honor the customer's preferred schedule. Weather or unsafe conditions may result in rescheduling with notice.
Cancellation. Either party may cancel with at least 48 hours' notice. Less than 24 hours' notice may incur a fee equal to one visit rate.
Property access. The customer agrees to provide safe and reasonable access to the service area on scheduled days.
Liability. The provider carries general liability insurance and is not responsible for pre-existing damage or conditions outside their control.
